The CTC will focus on delivering high value, comprehensive primary care to include acute and episodic care, chronic disease management, and wellness exams with a focus on whole health and prevention. The CTC will operate under a right place, right time, right modality framework, emphasizing proactive population health management.

Chief Purpose of this position: The Center for Team Care is seeking a physician with primary care experience to serve as a PCP for a panel of patients at a clinic in the metro Richmond area.

Position Responsibilities:

Clinical - 100%

  • Deliver high-quality, cost-effective and coordinated primary care with an uncompromising focus on patient experience.
  • Manage a patient panel by delivering acute and episodic care, conducting chronic disease management, and wellness visits.
  • Monday to Friday clinic hours with no nights, no call, no weekends.

Required Qualifications:

  • Board certification in Internal Medicine or Family Medicine
  • Direct outpatient experience in managing a complex panel in a busy primary care clinic
  • Demonstrated experience working in and fostering an environment of respect, professionalism and civility with a population of faculty, staff, and students from various backgrounds and experiences, or a commitment to do so as a faculty member at VCU

Preferred Qualifications:

  • Care management of pediatric patients
  • Experience in population health and proactive management of patient panels
  • Experience in value-based care or concierge care
  • Excellent verbal and written communication skills
